Please note:
- There is a $25 ID card replacement fee associated with this request.
- All requests must be accompanied by a valid (government-issued) piece of photo identification. Acceptable identification includes: birth certificate, driver’s licence, Certificate of Indian Status, permanent resident card, Canadian citizenship card, marriage certificate.
- Identification copies can be submitted to the Registrar's Office in person or by e-mail to transcripts@mta.ca.
