Change of degree request form

PLEASE NOTE

  • Students applying to change their degree program must apply to the registrar's office no later than the date specified for each program in the calendar of events
  • Students applying to transfer to the Bachelor of Commerce program must meet specific criteria outlined in section 11.5.14 of the academic calendar
  • Students applying to transfer to the Bachelor of Fine Arts program must submit a portfolio of artwork to the Fine Arts department

Before you begin to fill in this form, make sure you have:

  • student ID number
  • current majors/minors
  • new majors/minors
