U.S. William D. Ford Federal Direct Loan certification request form

Once you have completed your FAFSA application and you have decided to attend MtA, you must notify the financial aid office to request that your loan be certified. This request is made via the completion of the following form — U.S. William D. Ford Federal Direct Loan Certification Request Form for Mount Allison University.

Once this form is submitted, the financial aid and awards counsellor will be in touch regarding your requested loans.

If you make changes to your FASFA after submitting your request to MtA, you will need to notify MtA by emailing financialaid@mta.ca so we can determine if a reassessment is required.

Note: The financial aid and awards counsellor will reach out to each applicant with the maximums they are permitted to borrow for the loan period specified above. It is at this time that the applicant must confirm the value they wish to borrow. This value may be partial or full value of loans.

Application checklist and attestation statements

Please respond to each of the statements below. This section is required for Mount Allison to begin your U.S. Loan origination and disbursement process.

I have completed my application for Federal Student Aid (FASFA) for the academic year specified.

Are you a first time borrower?

Did you complete your required Entrance Counselling?

You must complete your entrance counselling before MtA can make the first disbursement of your loan. Entrance counselling is only required to be completed the first time you borrow. It remains active for the remainder of the borrowing years.

Have you completed your Master Promissory Note (MPN) for your new FASFA loan application?

MtA will not be able to distribute your loans until the MPN is completed. (Note: MtA requires all borrowers to complete a new MPN for each academic year.)

Will you be receiving any other external funding for the loan period such as scholarships, bursaries, grants, or loans?

(Example: Sallie Mae Loan, loan, $2,000 OR Legion Award, scholarship, $1,500)

Do you anticipate having exceptional costs beyond the typical cost of tuition, fees, books, room and board during this study period?

To be considered for exceptional costs, supporting documentation is required. Please submit supporting documentation to financialaid@mta.ca

Student declaration

I understand that by typing my name below, I irrevocably authorize Mount Allison University to withdraw money from my Student Financial Account (Connect) if the U.S. Department of Education requires a return of loan funds via Title IV Funds (U.S. Federal Direct Loan Funds), which may occur if I withdraw from Mount Allison during an academic term that I am currently borrowing money for.

Furthermore, if the return of Title IV Funds results in an outstanding balance on my Student Financial Account, I will pay the outstanding balance immediately. Any unpaid balance will be subject to late fees, loss of Mount Allison system services and potential collections as outlined in the University Academic Calendar.

I acknowledge that it is my responsibility to be aware of the U.S. Department of Education policies that are associated with Direct Loans, and I agree to abide by these regulations.
